Unrestricted Bequest
Specific Bequest
I hereby give, devise, and bequeath the sum of ________ dollars ($__________) to The University of Cincinnati Foundation, a nonprofit organization located in Cincinnati, Ohio (Federal Tax Identification Number 31-0896555), for the benefit of [the University of Cincinnati / UC Health], to be used for its general purposes.
Percentage/Residuary Bequest
I hereby give, devise, and bequeath ____ percent (___%) of the residue of my estate to The University of Cincinnati Foundation, a nonprofit organization located in Cincinnati, Ohio (Federal Tax Identification Number 31-0896555), for the benefit of [the University of Cincinnati / UC Health], to be used for its general purposes.
Restricted Bequests
If you would like your bequest to be used for a specific purpose, or to create a permanent endowment, please let us know. We would be happy to work with you to ensure your charitable objectives are met by creating a non-binding gift agreement that outlines the intended use of your gift. The Foundation will keep this agreement on record to help ensure your future gift is used in accordance with your wishes.

Beneficiary Designation
Since a will or trust typically does not dispose of certain assets—such as retirement accounts, insurance policies, bank accounts, or donor-advised funds—you may designate The University of Cincinnati Foundation directly as a beneficiary of these assets.
To do so, you will need to complete a beneficiary designation form provided by your financial institution. You will likely be asked to provide the following information:
